The victim, Tyler Jarrell, died of blunt force trauma injuries after being tossed 50 feet into the air when the ride broke apart. FOX8 Cleveland reports that four others remain hospitalized, including Jerrell's girlfriend, Keziah Lewis, 18.

The company said it conducted an investigation into the incident, which included a visit to the scene and a review of video footage of the incident.

"This finally led to the catastrophic failure of the ride during operation", Albert Kroon, product manager for KMG International BV, said in a statement Sunday.

The company that provided the ride to the Ohio State Fair, Amusements of America, described the Fire Ball as an "aggressive thrill" ride.

The company has now developed an "inspection protocol" to allow other rides of a similar type made by the Dutch firm to be checked out and reopened.

Ohio Department of Agriculture records showed passing marks on inspections for Fire Ball on about three dozen items, including possible cracks, brakes, proper assembly and installation. A similar ride at the Orange County Fair in California was also shut down to undergo a re-inspection after the OH incident, ABC station KABC in Los Angeles said.

The "excessive corrosion" was discovered inside the Fire Ball's gondola support beam during a "metallurgical inspection" of the ride, Kroon said. The Great Allentown Fair once featured a smaller version, but it is no longer in the fair's ride package, a state official said. His family has filed a wrongful death lawsuit.
